At Saint Francis Healthcare Campus, we encourage all patients to pre-register. This way, your paperwork can be prepared before arrival and your wait time will be minimized.
Items You Will Need for this Pre-Registration process:
- All active insurance cards
- Social Security numbers for patient and insurance subscriber
- Physician Order - information about your doctors, including names and addresses
- Emergency contact information
